As far as I know it's never been said on the show. But in the season 1 episode "The Benders" at the police station Sam and Dean's profiles were shown on the police database with their dates of birth, height, etc... - There were no middle name listed for either of them. - They've just always been Dean and Samuel "Sam" Winchester as far as I know. - Sorry that I couldn't give you a real answer!
